Apesar da sua promessa, as imunoterapias não conseguem tratar muitos cancros, incluindo mais de 80% de alguns dos cancros da mama mais avançados. E muitos dos pacientes que respondem ainda apresentam metástases. Uma nova pesquisa da Universidade de Stanford e do Arc Institute revelou uma maneira melhor de prever e melhorar as respostas dos pacientes.
Uma equipe liderada por Lingyin Li , professor associado de bioquímica em Stanford e Arc Core Investigator, descobriu que uma proteína chamada ENPP1 atua como um botão liga/desliga que controla a capacidade do câncer de mama de resistir à imunoterapia e de metastatizar. O estudo, publicado em 20 de dezembro no Proceedings of the National Academy of Sciences , mostrou que a ENPP1 é produzida por células cancerígenas e por células saudáveis dentro e ao redor do tumor, e que níveis elevados de ENPP1 nos pacientes estão ligados à resistência à imunoterapia e subsequentes metástases. A investigação poderá levar a imunoterapias novas e mais eficazes e ajudar os médicos a prever melhor a resposta dos pacientes aos medicamentos existentes.
“Nosso estudo deve oferecer esperança a todos”, disse Li, que também é bolsista do instituto Sarafan ChEM-H .
Descongelando tumores frios
As imunoterapias, como o pembrolizumab (Keytruda), funcionam bloqueando uma interação de amortecimento imunológico entre uma célula cancerosa e uma célula T, um tipo de célula imunológica. Para que isso seja eficaz, porém, as células T precisam permear o tumor. Os chamados tumores “quentes”, como os do melanoma e de um subconjunto do cancro do pulmão, são tratáveis através de imunoterapias, mas muitos outros, como os cancros da mama e do pâncreas, são “frios”, desprovidos de infiltração de células T.
Em sua busca para transformar tumores frios em quentes, Li começou com cGAMP, uma molécula que as células produzem quando seu DNA é danificado, o que acontece quando uma célula se torna cancerosa. Se deixado intacto, o cGAMP ativa uma resposta imunológica através do que é conhecido como via STING, que pode ajudar a aquecer um tumor. Li descobriu anteriormente que o cGAMP é exportado para fora das células, mas muitas vezes, antes de poder desencadear uma resposta, uma proteína chamada ENPP1 mastiga estes sinais moleculares de “perigo”. O ENPP1, ela propôs, ajudava a manter resfriados os tumores resfriados.
Altos níveis de ENPP1 se correlacionam com mau prognóstico em muitos tipos de câncer, mas a proteína pode realizar muitas ações no corpo, então Li decidiu determinar se sua capacidade de mastigar cGAMP está por trás de seu significado clínico.
Um interruptor liga/desliga
Li começou a colaborar com dois professores da Universidade da Califórnia, São Francisco: Hani Goodarzi, também investigadora principal do Arc Institute, e Laura Van't Veer, médica que lidera o ensaio I-SPY 2, um ensaio inovador sobre câncer de mama. Os níveis de ENPP1 variam naturalmente entre os indivíduos, por isso a equipe analisou os dados dos pacientes no ensaio I-SPY 2 para ver como as respostas ao pembrolizumabe variavam com os níveis de ENPP1 no momento do diagnóstico.
Os resultados foram surpreendentes. Pacientes com níveis elevados de ENPP1 apresentaram baixa resposta ao pembrolizumabe e alta chance de metástases. Aqueles com níveis baixos de ENPP1 tiveram uma resposta elevada ao pembrolizumabe e sem metástases. ENPP1 previu tanto a resposta à imunoterapia quanto a probabilidade de recaída.
Duas coisas ficaram subitamente claras: que o ENPP1 era crítico nas metástases, não apenas nos tumores primários; e que deveriam observar o ENPP1 em células saudáveis, não apenas em células cancerígenas.
“Usando os melhores bisturis moleculares desenvolvidos em nosso laboratório, fiquei animado para cavar mais fundo e descobrir exatamente como o ENPP1 tem uma influência tão dramática nos resultados clínicos”, disse Songnan Wang , estudante de MD-PhD em bioquímica, pesquisador da Arc e primeiro autor no papel.
Numa série de estudos com ratos, Wang provou que a remoção total do ENPP1 ou a eliminação apenas da sua capacidade de mastigação de cGAMP em células normais e cancerígenas produziu exatamente o mesmo resultado: diminuição do crescimento do tumor e diminuição das metástases. E a equipe provou que isso resultou diretamente da supressão da via STING. Eles encontraram um botão liga/desliga.
No topo da cachoeira
As vias imunológicas são frequentemente descritas como “cascatas” com uma série de sinais que desencadeiam ações posteriores que eventualmente levam a uma resposta.
“Para que os cânceres impeçam o sistema imunológico de detectá-los, eles precisam construir barragens que bloqueiem o fluxo do sinal”, disse Li. “Mostramos que a ENPP1 atua como uma grande barragem no topo da cachoeira.”
Isto significa que os médicos podem usar os níveis de ENPP1 para determinar melhor o tratamento apropriado para pacientes com câncer de mama. Significa também que os medicamentos que destroem a barragem ENPP1 podem tornar as terapias existentes mais eficazes – e vários inibidores da ENPP1 já estão em desenvolvimento clínico.
Embora este trabalho tenha se concentrado no câncer de mama, Li acredita que o ENPP1 desempenha um papel crítico em outros tipos de tumores “frios”.
“Espero inspirar os médicos que tratam cancros – incluindo cancro do pulmão, glioblastoma e cancro do pâncreas – a investigar o papel do ENPP1 nos resultados dos pacientes”, disse Li.
